- [19:48:54] patient: Darcel Williams My biggest issue is with severe urge incontinence. When I have to go, I cannot hold it. I am peeing full-on myself. When I’m out in public, I use two super tampons and 6 drop poise pad. When I am home, I keep a poise pad on. This happens all day and overnight. I do get up at least twice at night to urinate and have issues making to the toilet. Thank you for your help!

- [16:30:08] treatment PT: Rikki Markham Your kit is on its way and should arrive soon (I can't see the tracking from my end yet)! Here's our game plan to help you start seeing improvements in less than TWO WEEKS: 1. Getting started right away is key! A good rule of thumb is within 24 hours of kit delivery if possible. 2. For the best results, I recommend daily sessions for your first week to start. You can mix and match between pod and no pod sessions based on what feels right each day. Does this feel manageable? 3. Many members notice positive changes at our first milestone of 9 sessions. Let's aim to reach that in less than 2 weeks! Send me a 👍 if this plan works for you! Please let me know if you have any questions!

- [17:37:49] treatment PT: Rikki Markham Fantastic job getting started this weekend, Darcel! I can see you're doing great with the hold and agility exercises. The breathing coordination tends to be the trickiest part at first (as you probably remember) - it's all about syncing your diaphragm with your pelvic floor movements, practice will help! I also sent over some videos for review. I'll look forward to seeing your progress when I return next week!
- [18:56:25] patient: Darcel Williams Ok. Thank you!

2026-09-10 (Thursday)
- [19:08:41] treatment PT: Rikki Markham Hi Darcel! I'm Rachel, covering for Rikki while she's out this week, I've been following your great progress! But I have noticed the breathing coordination exercise has been consistently a bit challenging (which is totally normal early on!) so let's troubleshoot together: - Try taking a smaller, gentler breath in rather than a deep belly breath - a bigger inhale can sometimes create too much downward pressure and throw off the coordination. - Focus on a slow, complete exhale as you contract, rather than a quick squeeze. Give this a try on your next session and let me know how it feels!

- [18:46:34] treatment PT: Rikki Markham Hi Darcel! Great consistency this week, session #6 already and your overall performance keeps climbing to 85%! How did you feel the breathing tips worked for you? If you're ever finding the pod and app aren't reading your movements correctly, feel free to press pause and recalibrate. Sometimes that manual recalibration step is necessary as the pod or your body can shift and move ever so slightly, and that affects its readings!

- [02:41:57] patient: Darcel Williams Even though I’m doing well with the exercises at night, I wake up at least twice to pee when I sit on the side of my bed the pee comes very strong and I start to gush if I don’t have a pad on I’m in trouble even though I’ve been using the techniques that I’ve learned through this pod, it doesn’t stop the gushing, and I barely make it to the toilet on time. I am concerned I think I may have to go to a gyno urologist.
- [02:43:15] patient: Darcel Williams I failed to mention that I had a total hysterectomy back in 2018. I don’t have ovaries. I don’t have any female organs with this contribute to my issue.
- [20:38:33] treatment PT: Rikki Markham Thank you for sharing this detail, and I completely understand your concern with the nighttime gushing, that sounds really frustrating and disruptive to your sleep. 😓 It's great that you're already noticing the techniques helping during the day. For that first trip out of bed, try this: - Before standing up, do a few gentle contractions ("the Knack") while still sitting on the bed's edge, before you shift your weight up. - Move slowly and deliberately from sitting to standing, avoiding any sudden position changes that can trigger a strong urge. If symptoms continue to feel unmanageable despite these strategies, it's reasonable to loop in your physician to discuss further evaluation. In the meantime, let's keep building your strength and coordination, progress can take a few weeks to translate into these tougher moments!

- [17:55:22] treatment PT: Rikki Markham Hi Darcel! It's Rikki, I'm back in the office this week. Following up on what you shared about the nighttime urgency, since that sounds tough to manage. It can take a few weeks for you to see changes within your pelvic floor muscles, 4-6 weeks. So try not to get discouraged. Let's get you back to sessions this week!

- [18:10:31] patient: Darcel Williams Hi Rikki! Yes, I am using the tips Rachel has shared. However, the urgency is the same and I am concerned that it won’t get better without medical intervention. I have surgery coming up soon and concerned about what it’s going to be like while I am under. I am not sure if a catheter will be placed in me. I am continuing with my excercises. Thank you for reaching out. Have a blessed day!

- [18:15:18] treatment PT: Rikki Markham Hi Darcel! Thank you for the update. It's completely understandable to feel concerned about your upcoming surgery, that's a valid thing to want clarity on. Questions about catheter placement or surgical care are best directed to your surgical team, as they'll know the specifics of your procedure and can address that directly. In the meantime, keep up with your exercises as you're able, consistency is your muscles need right now for change to occur. When is your surgery scheduled for?
